PTS / NRP Form Requests 2024 - 2025 Season

AAA Permission to Skate U18
Forms will be emailed out to those requesting by an automated system AA once issued. The PTS form will be emailed to you prior to tryouts (enusre you enter an email below).
Only 1 NRP form per player to AA tryouts.
You must list the centre you wish to tryout out for. Once a NRP form is issued it is final and a 2nd form cannot be issued for AA.
